Life Threatening Health Conditions Asthma Allergy Diabetes Seizures
A life-threatening health condition is defined as "a condition that will put the child in danger of death during the school day if a medication or treatment order providing authority to a registered nurse and nursing plan are not in place."
Washington State law (RCW 28A.210.320 and 392-380 WAC) helps your student's school provide for the safety and health of children with life-threatening conditions.
State law requires children with life-threatening conditions to have a medication and/or treatment order on file and the necessary medications before or on the child's first day of attendance at school.
Life-threatening health conditions include: asthma, food and bee sting allergies, diabetes, seizures or a cardiac condition.
If your child has a life-threatening condition, please have a Healthcare Provider complete the required medication and treatment forms for your child's condition. All parents who have a child with a life-threatening condition are required to fill out an Authorization for Exchange of Medical Information form. Additional forms may be needed based on your child's health condition. All forms and medications must be provided to your child's school before the first day of attendance. The school nurse will contact you to develop an appropriate nursing plan.
